Gastroenterological Surgery department strives to meet the needs of our patients and contribute to regional medical care. Our organ-specific divisions, consisting of upper gastrointestinal, lower gastrointestinal, and hepatobiliary, pancreatic, and rectal sections, provide highly specialized surgical treatment for malignant diseases. Our experienced staff also treat benign conditions, such as esophageal achalasia, reflux esophagitis, gastroduodenal ulcers, cholelithiasis, and inguinal hernias, as well as emergency transport. We receive many patient referrals from within and outside Kanagawa Prefecture, and our surgical experience for esophageal, gastric, colon, pancreatic, bile duct, and liver cancers is among the best in Japan. We also perform robot-assisted surgery using the Da Vinci system, which allows for minimally invasive and precise surgery, on many patients with esophageal, gastric, rectal, colon, pancreatic, and liver cancer, making us one of Japan's leading hospitals.
Our department's motto is "from diagnosis to treatment," so that we can provide responsible medical care to referred patients. Unlike so-called centralized facilities, we place great importance on the entire process from diagnosis to treatment. Cancer treatment involves many methods, including endoscopic therapy, surgery, chemotherapy, immunotherapy, and radiation therapy. Multidisciplinary treatments, such as pre- and post-operative chemotherapy (radiation) therapy, have also become standard. By providing these diverse treatments in Gastroenterological Surgery, we are able to provide patients with accurate and seamless care. Even if cancer recurs after surgery, we strive to alleviate the anxiety of patients and their families by being involved in the treatment.
Patient needs are becoming more diverse, and the number of elderly patients with underlying diseases and cancers in other organs is increasing. In response to this historical background, we always consider "what is best" for each patient and their family, and strive to provide medical care. Furthermore, for patients who are difficult to treat at other facilities, we take advantage of our university hospital's advantages and provide safe surgical treatment under a cross-sectional cooperative system within the hospital. We also participate in many clinical trials and clinical studies to create new and standard treatments, and quickly provide the latest knowledge gained from such multi-institutional collaborative research.

Treatment details
Gastroenterological Surgery aims to be a medical department that is trusted by doctors at referring medical institutions.
For malignant esophageal diseases, we combine endoscopic examinations, gastrointestinal contrast examinations, CT, MRI, PET/CT, etc., and provide appropriate treatment based on accurate diagnostic results. For benign esophageal diseases, we have a specialized testing system in place, including esophageal pressure tests and pH/impedance tests, and we accept even benign diseases that are difficult to treat. Endoscopic surgery using minimally invasive thoracoscopes and laparoscopes is the standard surgery, and we also actively perform robot-assisted surgery.
We perform surgery, endoscopic therapy, and chemotherapy for gastric diseases, primarily targeting gastric cancer and gastric submucosal tumors. Our treatments are characterized by preserving the stomach as much as possible and reducing surgical invasiveness, resulting in a significant improvement in postoperative quality of life. For early-stage gastric cancer, we actively perform laparoscopic robot-assisted surgery. For terminally ill patients, we work with Department of Palliative Care to provide home care with total parenteral nutrition.
We perform approximately 200 colon cancer surgeries per year. For colon cancer, we basically perform laparoscopic surgery, minimizing pain and suffering while maintaining the cure for the cancer, and aiming for early social reintegration of patients. We are also expanding the application of laparoscopic surgery for rectal cancer. In particular, for locally advanced lower rectal cancer, we perform preoperative chemoradiotherapy, which is the international standard treatment, to improve treatment outcomes. In addition, we will introduce laparoscopic surgery widely for recurrent cancer, malignant diseases other than carcinoma, inflammatory bowel diseases such as ulcerative colitis and Crohn's disease, chronic intestinal obstruction, small intestinal diseases, and emergency diseases, aiming to improve treatment outcomes. We also specialize in surgery for pelvic recurrence and advanced malignant diseases.
The Hepato-Biliary-Pancreatic Surgery Department primarily performs surgical treatment and chemotherapy for hepato-biliary-pancreatic cancer. We also treat benign diseases such as cholelithiasis. We perform minimally invasive endoscopic surgery using laparoscopy in many cancer cases, and also perform robot-assisted surgery. We work closely with Gastroenterology and Department of Diagnostic Imaging to perform diagnostic imaging and endoscopic diagnosis, and select the optimal treatment after discussions at joint conferences between the Surgery, Gastroenterology, Department of Diagnostic Imaging, and Department of Diagnostic Pathology. Thanks to excellent teamwork between each department and a proven track record of treatment by experienced surgeons, our hepato-biliary-pancreatic surgery department has been certified as a high-level training facility A by the Japanese Society of Hepato-Biliary-Pancreatic Surgery.

Main medical achievements
1. upper gastrointestinal tract
The total number of treatments for esophageal diseases in 2023 was 221. There were 209 cases of esophageal cancer, of which 69 underwent surgery, and 159 underwent endoscopic treatment, stent insertion, chemotherapy, and chemoradiotherapy. In addition, we performed surgery on 8 cases of benign esophageal diseases, including submucosal tumors, gastroesophageal reflux disease, esophageal achalasia, and idiopathic esophageal rupture. Thoracoscopic surgery and laparoscopic surgery, which have small incisions and are less painful, are called endoscopic surgery and are performed in approximately 70% of esophageal cancer cases nationwide. Our department is a leading facility in Japan and performs almost all cases. In addition, we have introduced robot-assisted surgery since 2019 and are actively performing it. As an "Esophageal Surgery Specialist Certified Facility" by the Japan Esophageal Society and a "Certified Participating Facility" of the Japan Clinical Oncology Group (JCOG), we provide advanced medical care for esophageal diseases and contribute to the establishment of standard treatment.
Gastric diseases are mainly treated for gastric cancer and submucosal tumors (GIST, etc.). In the in-hospital cancer registry at our hospital in 2022, the total number of gastric treatments was 219, including 70 surgical cases. Chemotherapy (anticancer drug treatment) is performed for cases of unresectable metastasis and recurrence. Surgical treatment for gastric cancer is determined based on the location (site) and depth (depth of invasion) of the cancer, and the presence or absence of metastasis (lymph node metastasis and distant metastasis), and includes pylorus-side gastrectomy, total gastrectomy, and fundus-side gastrectomy. Partial gastrectomy is performed for gastric submucosal tumors. Surgical approaches include laparotomy, laparoscopic surgery, and robot-assisted surgery, and are selected according to the condition. In recent years, we have been actively performing minimally invasive surgery (robotic surgery and laparoscopic surgery) that is less burdensome for patients, and the number of such cases is increasing. Chemotherapy is performed for unresectable or metastatic/recurrent gastric cancer. For postoperative gastric cancer with a final diagnosis of Stage II/III, postoperative adjuvant chemotherapy is performed to prevent recurrence. In addition to surgery, we also use molecular targeted drug therapy to treat GIST.
2. lower gastrointestinal tract
Every year, we treat approximately 150 patients with colon cancer and approximately 60 patients with rectal cancer. Colon cancer is divided into stages I, II, III, and IV depending on the stage of progression, and recurrence occurs in 20% of stage II patients and 35% of stage III patients with surgery alone. In stage IV, treatment for metastasis to the liver, lungs, etc. must be performed at the same time. Therefore, we provide specialized treatment (called multidisciplinary treatment) according to the stage of progression of each patient. Specifically, we perform surgery using laparoscopy for large cancers and cancers other than those that have infiltrated the surrounding area, and radiation therapy before surgery for locally advanced rectal cancer. This reduces the recurrence rate in the pelvis, which is 15-20% with surgery alone, to 2-3%, and further shrinks the cancer to avoid the need for a permanent colostomy. We also specialize in surgery for pelvic recurrence and advanced malignant diseases. In 2021, we performed approximately 210 colon cancer resections, approximately 50 other malignant disease resections, and approximately 120 benign disease surgeries (approximately 80 inguinal hernia surgeries).
Recently, mainly in Europe and the United States, it has been reported that a treatment method that combines radiation therapy and chemotherapy for advanced rectal cancer (TNT: Total Neoadjuvant Therapy) can eliminate the need for surgery in about 30-50% of patients, and we have introduced this treatment in our department. To date, we have performed TNT on more than 20 patients, and about half of the patients have been observed without surgery. In order to fulfill our mission as a university hospital, we also participate in various clinical trials and clinical trials. We also put effort into mentoring young patients.
3. Liver, gallbladder and pancreas
Each year, we perform approximately 60 pancreatoduodenectomy/total pancreatectomies (including robotic-assisted surgery), approximately 20 distal pancreatectomies (including laparoscopic/robotic-assisted surgery), and approximately 70-80 liver resections (including laparoscopic liver resections and robotic-assisted surgery). In April 2024, we opened Pancreatic and Biliary Diseases Center focusing specifically on the diagnosis and treatment of this area. We perform numerous highly challenging pancreatectomies and liver resections, and actively provide preoperative and postoperative adjuvant chemotherapy to improve treatment outcomes. Hepato-biliary-pancreatic cancers are often difficult to treat, but recent advances in chemotherapy have led to an increase in the number of drugs available under health insurance. Even for cancers deemed difficult to resect at initial consultation, prior chemotherapy can sometimes offer a treatment strategy that allows patients to avoid abandoning resection. In addition, particle beam therapy for locally advanced cancer is also covered by insurance, and from a surgical perspective, we will present all of these treatment options and provide treatment methods that maximize the benefits of surgical treatment.
